The Ages & Stages Questionnaire

How can you give kids the best start in life? Complete regular questionnaires on your child's developmental and social-emotional growth to assure that your child is developing appropriately or to catch delays early! We can provide a questionnaire to you at anytime, during a playgroup, in a rural setting, just ask a staff member. As well, we are now able to have parents fill out these screens online on the Camrose FRC ASQ -3 and ASQ-SE -2 site. Parents can fill out this secure questionnaire and someone from our office will contact you to talk about your child's survey. Please click the link below to access online services.

What Is the Ages and Stages Questionnaire? The ASQ-3 is an assessment tool that helps parents provide information about the developmental status of their child young child across five developmental areas: communication, gross motor, fine motor, problem solving, and personal-social. These questionnaires are for children from 1- 66 months of age.

What Is the Ages and Stages Questionnaire - SE? The Ages & Stages Questionnaires® :Social Emotional (ASQ:SE) is a first level screening tool that is designed to identify children who may be at risk for social or emotional difficulties. The core of ASQ:SE is a series of 8 questionnaires that correspond to age intervals from birth to 6 years.
